Born into a devout Roman Catholic family, he consecrated himself to Jesus at age 5. At age 15 he joined the Capuchin order and took the name Pio in honour of St. Pius I. In 1910, the year in which he became a priest, he received the stigmata (bodily marks corresponding to the wounds suffered by the crucified Jesus) for the first time, though they eventually healed. He was drafted into the Italian military in 1915 for medical service during World War I but was shortly discharged because of his poor health.

Pio wrote in his letters that, early in his priesthood, he experienced bodily marks, pain, and bleeding in locations indicative of the (not yet visible) stigmata. In a letter to his spiritual companion and confessor Father Agostino Gemelli, dated 21 March 1912, Pio wrote of his devotion to the mystical body of Christ and the intuition that he would bear the stigmata.
From his youth, his health was not very robust, and especially pio padre in the last years of his life it declined rapidly. Sister Death took him well-prepared and serene on 23 September 1968 at the age of eighty-one. An extraordinary gathering of people attended his funeral.Â
